I would like to die at home – where can I record this?

If it is your wish to die at home then you can include this in your Advance Decision as it will help your healthcare team to understand your wishes.  However, anything you write in relation to where you are cared for will not be binding on healthcare professionals because only the refusal of medical treatment is legally binding in an Advance Decision.

You can also include your wishes for where you would like to be cared for or where you would like to die in the Advance Statement.
